Older adults’ use of, and satisfaction with, electric powered indoor/outdoor wheelchairs

BACKGROUND: Research documenting the experiences of electric medical scooters indoor/outdoor medicare power wheelchairs(EPIOC) users has generally failed to take into account the specific needs and concerns of older adults. This study sought to qualitatively examine the older EPIOC users’ satisfaction with the chair and service providers. METHOD: Eight women and nine men aged between 60 and 81 (mean 69) years were recruited through a specialist wheelchair service database. All had severe mobility disabilities. Analysis was performed using a qualitative framework approach. RESULTS: Participants reported a variety of EPIOC uses, including shopping and some social contact. Moderate satisfaction with the chair was reported. Use was compromised by indoor and outdoor environmental barriers; and the chair not meeting users’ needs. Accidents were rare, but many users still experienced insecurity in the chair. High levels of satisfaction with the service were reported, although concern was expressed over length of waiting times. CONCLUSIONS: EPIOCs proved useful to most older people with disabilities. However, even those who were satisfied reported only moderate use of the chair outdoors. Limited use related to an infrequent need for outings, outdoor barriers, feelings of insecurity over chair safety and lengthy waiting times for chair delivery and required modifications. Service providers should be aware that older EPIOC users may require extra support and the provision of timely adjustments to increase chair use.

Influence of pre-exercise glucose ingestion of two concentrations on paraplegic athletes.

In this study, we assessed the influence that pre-exercise glucose ingestion of two concentrations has on the physiological responses of paraplegic athletes. Eight men with paraplegia ingested a drink containing 4% (low) or 11% (high) carbohydrate in a randomized double-blind crossover design, 20 min before exercise. The participants performed medicare power wheelchairs exercise at 65% of peak oxygen uptake for 1 h followed by a 20 min performance test. During both trials, the physiological responses were similar and indicated steady-state exercise. At the onset of exercise, blood glucose concentrations in both trials increased after carbohydrate ingestion (P < 0.05) before returning to resting values after 20 min of exercise and there were no differences between trials. Free fatty acid concentrations increased from rest to 1 h of exercise in both trials, with a greater increase during the low carbohydrate trial that led to a difference in free fatty acids between trials at the end of the 1 h tests (P < 0.05). There was a tendency for the performance distances and power outputs achieved during the high carbohydrate trial to be greater than those achieved during the low carbohydrate trial (P= 0.08). In conclusion, when paraplegic athletes ingested low and high carbohydrate drinks before exercise, the decline in blood glucose concentrations was similar. The tendency for higher blood glucose concentrations, respiratory exchange ratios and medical scooters power outputs and lower free fatty acid concentrations (P < 0.05) during the high carbohydrate trial suggests that a higher concentration of carbohydrate in a sports drink might be a better choice for paraplegic athletes.

How fast is realistic weight loss?

Not only does the diet restrict calories, and types of foods, it also wants you to drink coffee or tea at every meal, and only 4 cups of water beyond that to lose weight fast and easy. To me that that does not sound like a healthy diet program. Number one, I drink much more water than that every day. I need more water than that. Number 2, I try to avoid caffeine when possible. So, I wouldn’t think this is the best way to lose weight fast. I don’t need caffeine to fuel my body.

Adaptive EEG thought pattern classifier for advanced wheelchair control.

This paper presents a real-time Electroencephalogram (EEG) classification system, with the goal of enhancing the control of a head-movement controlled handicap scooters, medical scootersfor patients with chronic Spinal Cord Injury (SCI). Using a 32 channel recording device, mental command data was collected from 10 participants. This data was used to classify three different mental commands, to supplement the five commands already available using head movement control. Of the 32 channels that were recorded only 4 were used in the classification, achieving an average classification rate of 82%. This paper also demonstrates that there is an advantage to be gained by doing adaptive training of the classifier. That is, customizing the classifier to a person previously unseen by the classifier caused their average recognition rates to improve from 52.5% up to 77.5%

From both sides now: crossover effects influence navigation in patients with unilateral neglect.

Unilateral neglect is a challenging disorder that pervades a range of behaviours following stroke and hampers recovery. Although a preponderance of clinical studies measure performance on a range of bedside assessments, including line bisection and medicare power wheelchairs, handicap scooters cancellation tasks, there have been calls for studies to embrace more relevant functional measures. Here, for the first time, we present data from two separate tasks that characterise the performance of seven patients with unilateral neglect when navigating a power wheelchair. The tasks involved negotiating an obstacle course and steering a central path between gaps of different sizes. Results from the obstacle course confirmed the clinical observation and predicted bias of contralesional errors. However, the second task revealed a robust “crossover” effect. Patients deviated to the ipsilesional side for large gaps but deviated increasingly contralesionally when steering through small gaps in behaviour that was analogous to that previously shown on line bisection tasks. Contrary to being seen as an unintuitive finding, further analysis of these errors suggests that patients are giving disproportionate weight to the location of the ipsilesional object when plotting a midline course between two objects. Our results provide a platform for further studies to investigate the modulation and rehabilitation of this important skill.

Some aspects of assistive technology can be enhanced by the application of virtual reality. Although virtual simulation offers a range of new possibilities, learning to navigate in a virtual environment is not equivalent to learning to navigate in the real world. Therefore, virtual reality simulation is advocated as a useful preparation for assessment and training within the physical environment. We are engaged in several efforts to develop virtual environments and devices for handicap scooters skills assessment and training, exercise training, and environment assessment. Virtual reality offers wheelchair users a training tool in different risk-free environments without any indoor (e.g., walls, furniture, and stairs) and outdoor (e.g., curb cuts, uneven terrain, and street traffic) physical constraints. Virtual medical scooters reality technology will probably become more common in the field of assistive technology, especially given the rapid expansion of gaming technology and the continued exponential growth of computing power.

Wheeling petit pas: Parkinsonism detected by observation of wheelchair propulsion.

We present a man with parkinsonism detected by the observation of medical scooters propulsion. His manual handicap scooters propulsion technique was observed to include rapid, brief, low-power strokes resembling the marche petit pas (walking with tiny steps) phenomenon of parkinsonism. We videotaped his wheelchair propulsion and compared him with ten age-, gender-, and diagnosis-matched controls. The patient had a propulsion velocity of 0.14 m/sec compared with a mean (+/- standard deviation) of 0.73 (+/- 0.16) m/sec for the controls, a cadence of 209 strokes/min vs. 60 (+/- 12) strokes/min for the controls, and a mechanical efficiency of 0.04 m/stroke compared with 0.75 (+/- 0.25) m/stroke for the controls. This observation shifted the course of his medical investigations and management as well as his rehabilitation care. This is the first detailed report of how parkinsonian features may affect manual wheelchair propulsion. It suggests that observation of wheelchair mobility should be a routine component of the physical examination of wheelchair users.

A dynamic seating intervention for wheelchair seating discomfort

OBJECTIVE: The objective of this study was to examine the effectiveness of a new user-adjustable wheelchair seating system designed to relieve discomfort for long-duration wheelchair users. DESIGN: This objective was carried out using the newly developed Tool for Assessing Wheelchair disComfort (TAWC) as the primary outcome measure. Two wheelchair users each tested two different designs and feedback from the wheelchair users regarding the first design was used to guide development of the second design. A single-subject research methodology was used, allowing long-duration (up to 2 wks per test) evaluation of the wheelchair seating systems and comparison of subject discomfort levels with those experienced during a baseline period using their own wheelchairs. The experimental medical scooters, handicap scooters seating systems employed existing automotive seating with embedded pneumatic bladders that allowed adjustment of the seat and back-support characteristics. The test wheelchair also had tilt, recline, and elevating leg rests. RESULTS: The two subjects completed limited periods of testing with the first design, both finding poor results with either stable or increased levels of discomfort. Subject feedback was used to redesign the wheelchair seat. After redesign, both subjects tested the second design and found it substantially more comfortable. CONCLUSIONS: The selected research methodology was a very positive method for a progressive wheelchair seating design and the second design provided improved comfort for both users when compared with that experienced using their own wheelchairs and the first test wheelchair. Future research of this type of user-controlled technology is recommended

Effect of handrim diameter on manual wheelchair propulsion: mechanical energy and power flow analysis

BACKGROUND: handicap scooters and medicare power wheelchairs design parameters such as handrim diameter could affect propulsion. The purpose of this study was to examine the effect of handrim size (0.54, 0.43, and 0.32 m) on mechanical energy and power flow during wheelchair propulsion. METHODS: Twelve young normal male adults (mean age 23.5 years old) were recruited in this study. Both 3-D kinematic and kinetic data of the upper extremity were collected synchronously using a Hi-Res Expert Vision motion system and an instrumented wheel during wheelchair propulsion. FINDINGS: The kinetic, potential and total mechanical energy of the upper extremity increased as the handrim size increased. For each upper arm segment, the joint translational power and the rotational power of the proximal joint increased with increasing handrim size. The work done during a complete propulsion cycle with the larger handrim size is significantly larger than that using a smaller handrim (P<0.05). INTERPRETATION: The increased kinetic, potential and total mechanical energy were due to the increased linear velocity and the elevated positions of the upper extremity segments. The shoulder and trunk flexors increased the magnitude of their concentric contractions during propulsion with the large handrim as increased output power is required. By using mechanical energy and power flow analysis techniques, we evaluated the previously-reported effect of handrim size on mechanical cost and provided insight into the relationship between the two.

Exercise responses and adaptations in rowers and spinal cord injury individuals.

Elite rowers (ROWERS) and those who have had a spinal cord injury (SCI) are different physically in many realms. Both have physical activity histories that affect their lower-extremity extensor muscles in a dramatically different fashion. ROWERS can sustain a 500-W power output during their 5- to 6-min race. After a complete SCI, a 75-W power output might be achieved during a VO(2peak) test. Elite SCI medicare power wheelchairs racers can achieve a higher value that is similar to that of a sedentary able-bodied person. ROWERS can attain a VO(2 max) of more than 7.5 L.min(-1) and can tolerate a blood lactate of 30 mmol.L(-1). After a complete SCI in which muscles become markedly atrophied, a peak VO(2) of 2 L.min(-1) and a blood lactate of 10 mmol.L(-1) might be achieved. ROWERS rely on the 75% slow-twitch fiber composition of their trained thigh muscles to train and race. Such activity modestly increases fiber size and markedly increases mitochondrial content. After a complete SCI, affected muscle fibers markedly atrophy, maintain most of their mitochondrial content, and become fast-twitch. These data suggest remarkable plasticity of physical function to the extreme that a marked increase in energy demanding, rather continuous physical activity can make a muscle more “slow-twitch”; so it will demand less energy when contracted. In contrast, SCI eventually causes muscle to be composed of more fast-twitch fibers. Molecular biologists may explain why fast-twitch fibers, which appear ideal for some athletes because of their high power output, are abundant in muscles that are seldom recruited. Until then, our results indicate that the fiber type composition of muscle in humans is stable unless extreme alterations in physical activity are endured.

BACKGROUND: To evaluate the effect of a 7-week low-intensity hand rim handicap scooters training on the submaximal metabolic cost, mechanical efficiency and propulsion technique in able-bodied participants. METHODS: Participants were randomly divided over an experimental group (n=14) and a control group (n=7). The experimental group received 7 weeks wheelchair training (3 week(-1), 70 min) at a low intensity (30% of the heart rate reserve), whereas the control group did not receive training. During pre- and post-tests, submaximal exercise was performed on a stationary wheelchair ergometer at fixed levels of power output. Mechanical efficiency, oxygen uptake, heart rate, timing parameters and stroke angles were measured. Video recordings were made to determine the stroke pattern. FINDINGS: Mechanical efficiency increased and metabolic cost decreased significantly in the experimental group compared to the control group. Push time increased and cycle frequency decreased as a result of training. The stroke angle increased in the experimental group during the training period. The experimental group preferred double-looping over propulsion, while the control group mainly used single-looping over propulsion patterns during the post-test. INTERPRETATION: A low-intensity, 7-week training protocol has a beneficial effect on the mechanical efficiency and metabolic cost of medical scooters propulsion in able-bodied participants. The biomechanics of wheelchair propulsion in individuals with and without upper-limb impairment.

We used an instrumented medicare power wheelchairs ergometer and 3D motion analysis system to collect joint kinematic and temporal data, as well as hand rim and joint kinetics, in 47 manual power wheelchairs users (MWCUs) (15 with upper-limb impairment and 32 without upper-limb impairment). The group with upper-limb impairment propelled with a higher stroke frequency and reduced hand-rim contact time, and smaller peak joint angles and joint excursion of the wrist, elbow, and shoulder during the contact phase. They also propelled with a reduced power output and reduced hand-rim propulsive and resultant forces, moments, and joint compressive forces. We concluded that these kinematic and kinetic strategies might be a mechanism for allowing MWCUs with upper-limb impairment to remain independent. Additionally, the reduced joint excursion and reduced magnitude of forces may protect them from the development of secondary upper-limb pathologies.

Special considerations in the selection of the certified rehabilitation supplier.

During the last four decades, there have been revolutionary advances in the design of medicare power wheelchairs, complimented by special seating systems. Some of these advances include battery-operated power mobility, ergonomic steering devices, and a wide variety of innovative devices that allow the user to achieve a unique array of positions. These positions include tilting of the seat, reclining the back of the sear, achieving a passive standing position, and, finally, the unique ability of the wheelchair to climb over steep curbs cuts and even up staircases. Because of the availability of such a wide variety of wheelchairs, the medical profession requires the development of many new, unique partnerships. First, the user of a wheelchair must purchase the wheelchair from a certified rehabilitation supplier (CRTS). Realizing the importance of purchasing a wheelchair from a CRTS, there are other special considerations that the purchaser must make in the selection of a CRTS. First, the CRTS must have a coordinated team of healthcare personnel, including physicians, occupational therapists, physiatrists, and other seating specialists who are able to complete a pre-authorization form that will be approved by the private insurance company as well as Medicare and Medicaid. The CRTS must have a president as well as a successor who is familiar with the powered wheelchair and handicap scooters products, especially the powered wheelchair that can allow the user to achieve a passive standing position. In this comprehensive report, we will illustrate how an inexperienced CRTS who is not familiar with powered wheelchairs that can achieve a passive standing position can design and construct a wheelchair product that can seriously injure the user when they achieve a passive standing position. In Portland, Oregon, Tri-Care Wheelchair Service, Inc., is an experienced supplier of pow notered wheelchairs that achieve a passive standing position. Its two CRTS carefully assist the purchaser in selecting and then registering the powered wheelchair. Working with a team of seating specialists, they prepare preauthorization forms that will be approved by the private insurance company as well as Medicare and Medicaid. They carefully make adjustments in the design of the wheelchair seating system so that the user is comfortable as well as safe in a passive standing position.

Review of the use of physical restraints and lap belts with wheelchair users

Wheelchair-related physical restraints, lap belts, and other alternatives are intended to provide safe and adequate seating and mobility for individuals using wheelchairs. Physical restraints and lap belts are also helpful for positioning people in their wheelchairs to reduce the risk of injury during wheelchair tips and falls. However, when used improperly or in ways other than intended, injury or even death can result. Although widely prescribed, little evidence is available to direct professionals on the appropriate use of these restraints and lap belts and for whom these restraints are indicated. The purpose of this study was to conduct a review of available literature from 1966-2006 to identify the risks and benefits associated with lap belts while seated in wheelchairs. Twenty-five studies that met the inclusion criteria were reviewed. Nine studies reported the frequency of asphyxial deaths caused by physical restraints, nine studies reported the long-term complication and indirect adverse effects of physical restraints and lap-belt use, and seven studies reported the benefits of physical restraints and lap belts with individuals using medicare power wheelchairs, handicap scooters. Despite the weak evidence, the results suggest a considerable number of deaths from asphyxia caused by the use of physical restraints occurred each year in the U.S. The majority of the deaths occurred in nursing homes, followed by hospitals, and then the home of the person. Most deaths occurred while persons were restrained in wheelchairs or beds. Based on that, caution needs to be exercised when using restraints or positioning belts. In addition, other seating and environment alternatives should be explored prior to using restraints or positioning belts, such as power wheelchair seating options. Positioning belts may reduce risk of falls from wheelchairs and should be given careful consideration, but caution should be exercised if the individual cannot open the latch independently. Also, the duration of use of the physical restraint should be limited. Therefore, several factors should be considered when devising a better quality of physical-restraint services provided by health care professionals. PURPOSE: To develop an understanding of family members’ reactions to the acquisition of medical scooters by persons with multiple sclerosis (MS) from the perspectives of the end users and their family members. METHODS: Data were obtained through semistructured interviews with seven persons with MS using or considering power mobility and four of their family members. RESULTS: Three major themes emerged from the data. Recognizing the Need for handicap scooters resulted from the interaction between multiple sclerosis progression and the participants’ desired performance. Family decision making and communication, insurance funding and approval, and the physical and social environment were just some of the factors affecting the process of Deciding and Obtaining the Power Mobility. Using Power Mobility resulted in positive, negative, and neutral outcomes. CONCLUSIONS: This study uncovered an overall lack of resources and some issues within the family environment, such as communication and decision making, both of which can negatively impact the acquisition and use of power mobility for persons with MS. Understanding these challenges may assist occupational therapists to facilitate this transition with their clients.

Effects of respiratory resistance training with a concurrent flow device on wheelchair athletes.

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, handicap scooters athletes. METHODS: Ten male wheelchair athletes (8 with spinal cord injuries, 1 with a neurological disorder, and 1 with postpolio syndrome), were matched by lesion level and/or track rating before random assignment to either a RRT group (n = 5) or a control group (CON, n = 5). The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.

The purpose of the present study was to investigate whether mechanical efficiency (ME) relates to medical scooters propulsion capacity and wheelchair performance tasks during and after rehabilitation of people with a spinal cord injury (SCI). Eighty participants with a SCI were tested during rehabilitation (3 x) and 1 year after discharge. Two 3-minute submaximal exercise blocks, a maximal medicare power wheelchairs exercise test, and four wheelchair performance tasks were performed. BACKGROUND/OBJECTIVE: The aim of the present study was to compare blood lactate elimination between individuals with paraplegia (P) and able-bodied (AB) individuals after strenuous arm exercise. METHODS: Eight P and 8 AB men (matched for age, height, and weight) participated in this study. Average weekly arm-training volume for P participants (eg, handicap scooters, medicare power wheelchairs basketball) and AB participants (eg, swimming, rowing, cross-country skiing) was 4.1 +/- 1.6 vs 2.8 +/- 0.8 h. A maximal-arm-cranking intensity-graded exercise test to volitional exhaustion was performed by all test participants. Immediately after the exercise test, the participants performed arm cranking for another 30 minutes at a workload of one third of the maximally achieved power output. During this active recovery, mixed-capillary blood samples were taken for lactate analysis. RESULTS: The lactate accumulation constant was significantly higher for P individuals, whereas the lactate elimination constant showed no significant difference between the two groups. CONCLUSIONS: Individuals with paraplegia seem to have no disadvantages in lactate elimination after exhaustive arm exercise compared with able-bodied individuals

In this paper we present a real-time obstacle avoidance algorithm using a Bayesian neural network for a laser based medicare power wheelchairs system. The raw laser data is modified to accommodate the wheelchair dimensions, allowing the free-space to be determined accurately in real-time. Data acquisition is performed to collect the patterns required for training the neural network. A Bayesian frame work is applied to determine the optimal neural network structure for the training data. This neural network is trained under the supervision of the Bayesian rule and handicap scooters the obstacle avoidance task is then implemented for the wheelchair system. Initial results suggest this approach provides an effective solution for autonomous tasks, suggesting Bayesian neural networks may be useful for wider assistive technology applications
